Child and Adolescent Case Management

This program provides community-based case management to child and adolescent survivors and their families, who reside in the Brain Injury Services of SWVA service area.

Case management is the process of identifying, planning, coordinating, monitoring, and evaluating services and resources to meet the individual's and family's needs. A collaborative process is used to identify family-centered goals, and marshal (coordinate) resources to meet those goals.

In addition, case management includes advocating for and creating community resources needed to assist survivors in maximizing recovery, optimizing employment or service potential, and reintegrating into their families and communities.

There is no cost for case management services directly provided by a Case Manager. If a person receives a service that is contracted for by their Case Manager, then the Financial Eligibility Form will determine whether the client will be required to contribute towards the cost of a particular service.
