Child and Adolescent Case Management

This program provides community-based case management to child and adolescent survivors and their families, who reside in the Brain Injury Services of SWVA service area.

Case management is the process of identifying, planning, coordinating, monitoring, and evaluating services and resources to meet the individual's and family's needs. A collaborative process is used to identify family-centered goals, and marshal (coordinate) resources to meet those goals.

In addition, case management includes advocating for and creating community resources needed to assist survivors in maximizing recovery, optimizing employment or service potential, and reintegrating into their families and communities.

Areas which are frequently the focus of case management services include:

• Behavior / Mental Health
• Education and School Re-entry
• Health and Medical Services
• Recreation and Socialization
• Respite Care
• Transportation

In addition to case management services, Brain Injury Services of SWVA provides:

• Advocacy
• Community Transition Services
• Education, Consultation, and Training to Local Schools and Service Providers
• Family Support
• Information and Referral
• Life Skills Training
• Professional Consultation
• Promotion of Brain Injury Prevention

There is no cost for case management services directly provided by a Case Manager. If a person receives a service that is contracted for by their Case Manager, then the Financial Eligibility Form will determine whether the client will be required to contribute towards the cost of a particular service.

Brain Injury Services is committed to streamlining delivery of services through interagency collaboration. Utilizing existing organizations provides cost-effective services to the individual and the community. Services contracted or coordinated through partnerships include:
